1) Colonic polyp:

polyp in colon      sonography of the ascending colon

ultrasound image of polyp in the colon      doppler imaging of polyp of colon

CT SCAN image of the polyp (below):

color doppler imaging of colonic polyp   CT SCAN image of the polyp

CLOSE-UP view of the CT image:                             Colonoscopic view of the polyp (below):

Zoomed CT scan image of colonic polyp    Colonoscopy image of the polyp

This 55 yr. old male patient underwent routine sonography of the abdomen. The scan reveals a rounded, solid, soft tissue lesion (isoechoic with the colon) of size 17 x 10 mm, in the lumen of the ascending colon. An atypical target sign is also seen. Color doppler shows a feeder vessel supplying the mass lesion. These ultrasound images suggest colonic polyp. Sonographic images taken using a Toshiba Powervision color doppler machine, courtesy of Dr. Ravi Kadasne, UAE. The polyp was also visualized on CT scan (see images above) and also on colonoscopy (above).

2) Sonography of Colitis:

multiple enlarged mesenteric lymph nodes     thickened colon

gross lymph node enlargement     thikened transverse colon

round worm infestation     associated appendicitis

The above ultrasound images of a child show: 1) multiple enlarged mesenteric lymph nodes (lymph node megaly)     2) gross thickening of the wall of the colon (almost the entire colon seems to be involved) 3) concurrent appendicitis 4) evidence of round worm infestation. This group of ultrasound findings suggest colitis (of infective etiology) with mesenteric lymph node enlargement. Sonographic images courtesy of Dr. Gunjan Puri, Surat, India, who used a Toshiba Xario color doppler and ultrasound system.

Case-2: Colitis:

longitudinal section of descending colon    transverse section ultrasound image of colon

Color Doppler image of colitis    note the marked thickening of the large bowel wall

The above ultrasound images show significant large bowel (colon) wall thickening (10mm.) both in transverse and longitudinal section of the descending colon. The Color Doppler image shows marked vascularity of the bowel wall. Such findings, especially the marked thickening of the colonic wall suggest inflammation, (colitis), even possible Chrohn disease of the colon. Ultrasound images are courtesy of Shlomo Gobi, Israel.

3) Fecaloma or impacted fecal mass:

fecal impaction in colon      ultrasound image of fecaloma

This elderly male patient presented with constipation and left iliac mass, which was hard on palpation. Sonography of the abdomen revealed a large, echogenic, rounded mass lesion occupying the colonic lumen. The anterior border of the mass appeared hyperechoic with no through transmission. These ultrasound images are diagnostic of fecaloma or impacted feces in the colon. Further hardening with calcium deposition and fecal stone (fecolith) formation are the final outcome of this condition, if left untreated. These images were taken using a Philips IU 22 color doppler machine, courtesy of Dr. Ravi Kadasne, UAE.

4) Colorectal cancer/ carcinoma of sigmoid colon:

sagittal section of the mass    mass involving the sigmoid colon

power Doppler image of colonic malignancy   sagittal and transverse ultrasound images of rectosigmoid cancer

This patient has a large mass (6 x 5.7 cms.) just above the urinary bladder with a central hyperechoic area. This is the target sign suggestive of mass (carcinoma) of the colon. The location suggests sigmoid/ rectosigmoid carcinoma. The Power Doppler image shows minimal vascularity in the mass. These ultrasound images suggest sigmoid cancer (carcinoma). The sigmoid colon is the part of the large bowel between the descending colon and the rectum and is one of the sites where malginancy is known to occur. On further imaging (CT scan study), it was found that some invasion of the urinary bladder had also occurred.
